Need some more info, like was it at startup,server up and running,editing, or touched your right ear. Also what happens after helps too.

server up,and running for couple of weeks. I installed the lair mod yesterday, and it started last night. It hasn't happened since this morning, but I been holding my left ear.

Any time. Morning noon or night. This time it shot 3 violations, but didn't shut down.

I really have no clue but to me it looks like bad memory calls. Did you do an offline recovery after you install the mod?

I don't think these are mud errors, and probably general WG errors. If I had to guess, possibly a permissions issue?

It does an offline recovery after each incident. I had to leave town for a neo surgeon consult, and when I got home it had done it again. First time since early yesterday morning.
Each time it happens I have to manually shut down wgs, and wgappo before anyone can log in. It does an offline mud recovery each time also.

Too my untrained eye it calling and trying to read from a memory address it can't access. Either it corrupt, wrong info, or the memory address doesn't exist.
