This ability designates a monster to guard the monster being edited. NMR says that multiple monsters can be set as guard, but this is not true. The game will only recognize a single entry. For example, if you have, from top to bottom in the abilities list, monsters 1, 3, and 2 set to guard monster 4, the only one that will be recognized in game is monster 2. If you have monsters set to guard monster 2 the effect does not waterfall down, meaning monster 1 would not guard monster 2 if it were set to when you attack monster 4.
Conclusion: you can only set a single guard using ability (146) per monster and the effect is only valid for the monster the player is attempting to engage.

You confused me on that one. Or maybe I'm just high. But my understanding you can have it on multiple monsters, but only 1 at a time will actually guard them. But been awhile since I played with monster guard.
