This is a stupidly long post, so I'm just gonna answer it a chunk at a time, sorry if I miss some call back to an earlier section, but its much easier to answer 1 question at a time.
I'm guessing that most if not all of these have hidden/special functions.
Some of them do, but most of them ...

Re: Monster Attack Calculations / Formulas
So I am trying to test between round casts now to make sure they work as I expected them to (and that they didn't have any affect on the mob's attacks [they don't seem to]).
Can anyone make any sense of this?
Ya sure. Do you want to give us any more information, like what this test actually was ...

Re: Monster Spells - Spell Attack Type -> Cast %
Actually, another piece of info, but irrelevant for the sake of sim'ing, is mud actually goes through the entire attack calculation, pulling the messages and everything, before checking to see if it has the energy to do it :P
So this actually isn't bad on their part, per se. One reason for this is ...

Re: Formulas / Calculations for AC and MR?
it checks ac first, then dodge
I was always told it checked dodge first. Learn something new everyday.
I was under the impression that dodge was calculated first as well, but that was simply hearsay from what other people have told me.
Seems like ti would make more sense if it were first ...

Re: Formulas / Calculations for AC and MR?
edit: Also of note: It looks like the resist may be calculated before the failure rate of the spell itself. But more importantly for the monster damage thing I'm working on, is that on a resist, half the energy spent to cast it is also returned, as if the monster failed to cast it.
Chance to ...

He's talking about some occations where the displayed damage doesn't match the actual damage taken.

also its D*D, and A*A
Yea I realized that after I posted but it still doesn't make it work out.
150*150 = 22500
300*300 = 90000
225000/90000=2.5
2.5/100 = 0.025 which is only 2.5% chance to miss. It should be around 35%.
Oh maybe I've f'd something up in my head... follow me here

Re: Formulas / Calculations for AC and MR?
Ya you should be getting 98% in testing. It caps. Guess I coulda mentioned that.
also its D*D, and A*A
There is a rather narrow range of values for AC and ACC that don't return the cap'd results -- This is one reason why high level combat is also always 5 misses or 5 hits.
