Need help with item DB issue

On the board I play on, the item list is completely fucked up.
If you do a "sys list lim" it lists all items, but it displays multiple entries.

For example, this is what it displays for the golden belt:

Item golden belt User APlayer
Item golden belt User APlayer

It does this for every limited in the list. They are all listed multiple times. The limited count has not been changed.

This is causing a major issue in the game. Whenever someone dies, their limited items poof and get returned to the monsters.

Has anyone ever experienced this? Does anyone have any kind of solution? I fear the sysop will have to do a full reset to fix this problem.

If anyone else runs into this. A re-index from the sysop menu solved this problem.
